Registered Nurse - Urology Practice in Chappaqua, NY


The Registered Nurse performs an age-specific plan of care for designated patients using the nursing process of assessment, diagnosis, outcome identification, planning, implementation, and evaluation. They collaborate with physicians and healthcare team members to coordinate procedures and treatments, demonstrating leadership and clinical judgment in directing patient care activities.

Responsibilities:

  1. Conducts patient interviews, explains policies and procedures to patients/significant others, reviews charts, assesses health status (gastrointestinal, cardiovascular, respiratory, renal, neurological), mobility, sensory deficits, prostheses use, and skin condition. Adjusts care plans as needed, assesses pain levels and management, and documents patient's physiological health status and care plans.

  2. Assesses psychosocial health status, perception of medical/nursing care, and expectations. Determines coping mechanisms, knowledge levels, cultural/ethnic needs, and ensures patient satisfaction. Documents psychological status, care plans, and coordinates support groups, counseling, and social services.

  3. Identifies patient outcomes, develops criteria for measurement, identifies actual/potential problems, and assesses educational needs based on psychosocial and developmental assessments. Establishes patient goals and outcome statements.

  4. Identifies care activities, prioritizes to achieve outcomes, coordinates cost-effective use of supplies, equipment, and medications. Documents care plans, collaborates with physicians and team members.

  5. Implements care plans, maintains patient safety, ensures safety precautions (side rails, call lights), performs procedures safely, reassesses patients, administers treatments, uses equipment correctly, provides emotional support, applies scientific principles, observes conditions, reports and records interventions accurately. Maintains documentation per policy, manages emergencies effectively, communicates with families/caretakers, plans discharge, coordinates referrals, provides discharge instructions, evaluates care outcomes.

  6. Measures care effectiveness, documents outcomes, performs variance analysis for improvement, designs/evaluates care systems, maintains accurate documentation.

  7. Collaborates with care team members, provides direction, collaborates with physicians on care plans.

  8. Implements prescribed treatments/procedures, keeps team informed of patient changes, uses clinical judgment in delegation, ensures timely completion of assignments.

  9. Performs skin care procedures (grade I-IV Decubitus Care, preventive care, dressings, compresses, irrigations) per facility procedures.

  10. Administers medications safely, identifies medication actions/dosages/side effects, prepares solutions, educates patients/significant others on medications/food interactions.

  11. Educates patients/families on conditions, communicates assessment data to care team/agencies, utilizes teaching opportunities/resources, plans early discharge, makes referrals, evaluates teaching effectiveness through feedback/documentation.

  12. Performs related duties as required. All responsibilities are essential functions under the Americans with Disabilities Act.

Requirements:

  • Graduate from an accredited School of Nursing.
  • Current Registered Nurse (RN) license in New York State.
  • Bachelor’s Degree in Nursing preferred; must be enrolled in an accredited BSN program within two years and obtain BSN within five years of job entry.
  • Specialized certifications as required (e.g., urology experience, surgical practice background).
  • Minimum [number] years of nursing experience preferred, with urology or surgical practice experience highly desirable.
  • BLS/CPR certification required; ACLS preferred.
  • Excellent communication, interpersonal, and organizational skills.
